This Privacy Policy applies to information we collect through our website, scheduling forms, phone/email/SMS communications, and in-clinic services. For protected health information (PHI), our HIPAA Notice of Privacy Practices governs. If there is any conflict, the HIPAA Notice controls for PHI.

  • Identifiers & contact details: name, email, phone, address, date of birth.
  • Appointment & account data: scheduling history, forms you submit, preferences.
  • Health & clinical information (PHI): history, symptoms, exam findings, diagnoses, care plans, imaging, referrals.
  • Insurance & billing: insurer, claim numbers (including MVA), policy details, transactions.
  • Device & usage: IP address, browser type, pages visited, timestamps, and cookies/analytics data (see §7).

Sources: you (forms, calls, emails, SMS), your other providers (with authorization where required), insurers/adjusters for claims, and service vendors (e.g., scheduling or payment processors).

3) How We Use Information

  • Treatment, payment, and operations: to provide care, coordinate with other providers, verify benefits, and run our practice.
  • Scheduling & communications: appointment confirmations, reminders, intake links, follow-ups.
  • Customer support: respond to inquiries and requests.
  • Security & compliance: prevent fraud/misuse; meet legal/recordkeeping obligations.
  • Website performance & improvements: analytics to understand usage and enhance our site.
  • Marketing (limited): with your consent where required; you can opt out anytime.

4) When We Share Information

  • Service providers / “business associates”: scheduling, EHR, billing, payment, messaging, analytics, IT. Where applicable, we have agreements requiring appropriate safeguards.
  • Insurers & adjusters (including MVA): to verify coverage, support claims, and process payment.
  • Other providers: for coordination of care, with your authorization where required.
  • Legal & safety: to comply with law, subpoenas, court orders, or to protect rights, safety, and security.
  • Business changes: as part of a merger, acquisition, or other transition, subject to applicable protections.

We do not sell your personal information and we do not share it for cross-context behavioral advertising.

5) Communications

By providing your email or phone, you consent to receive messages related to your care, scheduling, and clinic updates. Standard carrier rates may apply for SMS. You can opt out of non-essential emails/SMS at any time (reply STOP for SMS or use email unsubscribe).

6) Data Retention

We keep records as required by law and professional guidelines. Non-PHI website data is retained only as long as needed for the purposes in this policy, unless a longer period is required by law.

7) Cookies & Analytics

Our site may use cookies and similar technologies (e.g., local storage, pixels) to enable core functionality, measure traffic, and improve performance. You can control cookies via your browser settings. Blocking some cookies may impact site features.

  • Essential: required for basic site and security functions.
  • Performance/analytics: aggregated usage insights (e.g., page views, dwell time).

Do Not Track: many browsers offer “DNT”; we currently do not respond to DNT signals.

Our website is not directed to children under 13. We obtain appropriate consent from a parent/guardian for the care of minors as required by law.

11) International Visitors

We operate in the United States. If you access our services from outside the U.S., you understand your information may be processed in the U.S., where laws may differ from those in your country.
