By accessing our website, scheduling care, or receiving services at Blyss Chiropractic, you agree to the following terms. Please read carefully.

1) Acceptance of Terms

These Terms of Service (“Terms”) govern your use of our website, scheduling system, communications, and in-clinic services. If you do not agree, please do not use our services.

2) Not Emergency Care

For medical emergencies, call 911 immediately. Our site and scheduling tools are not for urgent or emergent conditions.

3) Eligibility & Accurate Information

  • You must be able to consent to care (or have a legal guardian/representative who can do so).
  • You agree to provide accurate, current information when scheduling or completing intake forms.

4) Appointments, Late Arrivals & Cancellations

  • Arrival: Please arrive a few minutes early to maximize your appointment time.
  • Late arrivals: Sessions may be shortened to end on time; full fee may still apply.
  • Cancellations: Kindly provide at least 24 hours’ notice to cancel or reschedule. Missed/late-cancel appointments may incur a fee.

5) Financial Policy & Insurance

  • Payment responsibility: You are responsible for all charges not covered by insurance, including copays, deductibles, and non-covered services.
  • Estimates: Any benefit verification or estimate is not a guarantee of coverage or payment.
  • Balances: Balances are due upon receipt; overdue accounts may be subject to late fees where permitted.

6) Motor Vehicle Accident (MVA) & Third-Party Billing

  • We may bill auto insurers when appropriate and permitted. You agree to provide complete claim details and cooperate with documentation requests.
  • Depending on jurisdiction/insurer, an assignment of benefits and/or lien may be required to bill third parties directly.
  • If a claim denies or terminates, any remaining balance becomes your responsibility.

7) Communications Consent

  • By providing your email or phone, you consent to receive messages related to scheduling, reminders, intake, billing, and care coordination.
  • Standard carrier rates may apply for SMS or calls. You can opt out of non-essential messages at any time.

8) Telehealth (If Offered)

  • Telehealth may involve limitations compared to in-person examinations. We may advise in-person follow-up when clinically indicated.
  • You are responsible for having a private, safe space and adequate connectivity for telehealth visits.

9) Privacy & Health Information

We protect your health information in accordance with applicable law. See our Privacy Policy/Notice for details on how we collect, use, and disclose information.

10) Informed Consent to Treatment

Chiropractic, massage, naturopathic and related services involve benefits and potential risks. Examples include temporary soreness, stiffness, or—rarely—more serious adverse events. Your clinician will review options, answer questions, and you may accept or decline any procedure at any time.

11) Patient Responsibilities

  • Provide complete, accurate health history and updates about changes (medications, new symptoms, test results).
  • Follow home-care guidance as advised and communicate any concerns promptly.
  • Treat staff and other patients with courtesy and respect.

12) Website Content & Intellectual Property

All site content (text, graphics, images) is for general information and is owned by or licensed to us. You may not reproduce, distribute, or modify content without permission. Website information is not medical advice.

13) Third-Party Links & Tools

Links to external sites (e.g., scheduling, payments, resources) are provided for convenience. We are not responsible for their content, availability, or security.

14) Accessibility

We aim to provide an accessible experience. If you encounter a barrier, please contact us so we can assist.

15) Disclaimers & Limitation of Liability

  • Services are provided as permitted by law and within scope of practice. Outcomes vary by individual.
  • To the fullest extent allowed by law, we disclaim warranties of any kind related to the website and limit liability for indirect or consequential damages.

16) Governing Law

These Terms are governed by the laws of the State of Oregon, without regard to conflict-of-laws principles.

17) Massage Therapy Additional Terms

Purpose & Benefit
Massage therapy may reduce muscle tension and pain, improve circulation and mobility, and support relaxation and recovery.
Risks & Contraindications
Temporary soreness, light bruising, or fatigue can occur. Massage may be inappropriate with fever, contagious illness, uncontrolled hypertension, recent surgery, active infection, DVT/blood clot, or certain skin conditions. Tell us about any new or changing health conditions before we begin.
Your Choices During the Session
You may stop or change pressure/technique at any time. You may exclude any body area from treatment. Professional draping is required.
Scope & Professional Boundaries
Massage at Blyss is strictly therapeutic and non-sexual. Inappropriate behavior ends the session immediately and is fully billable.
Medical Care & Emergencies
Massage is not a substitute for medical care. Seek emergency care when appropriate. Inform us of pregnancy, injuries, medications (especially blood thinners), implanted devices, or recent procedures.
Allergies & Sensitivities
Tell us about allergies (oils/lotions/latex) and fragrance sensitivity. We aim to keep the space low-fragrance. Illness Policy If you’re ill (fever, vomiting, significant respiratory symptoms), please reschedule. We’ll do the same if your therapist is ill.
Minors
Clients under 18 require a parent/guardian to consent; a guardian may be asked to remain present.
Arrival & Late Policy
Arrive 5–10 minutes early for intake. If you arrive late, your session may be shortened to end on time and the full fee may apply. Cancellations & No-Shows Please give 24 hours’ notice to cancel or reschedule. Late cancellations may incur a 50% fee; no-shows may be charged 100% of the booked service.
Payment & Insurance
Payment is due at time of service unless pre-authorized. If using insurance, benefits must be verified in advance; otherwise, we can provide a receipt for your reimbursement claims.
Privacy
Your information is kept confidential and used for care coordination and scheduling in accordance with applicable privacy laws.
Consent
By booking, you confirm you’ve read and understand the above, you will disclose relevant health information, and you consent to receive massage therapy at Blyss..

18) Changes to These Terms

We may update these Terms from time to time. Continued use of our site or services after changes are posted constitutes acceptance of the revised Terms.
